A few things worth knowing ahead of your appointments.
Hours of Operation
Monday, Tuesday, Wednesday, Thursday: 6:00pm – 7:30pm
Sunday: 3:00pm – 4:30pm
Cancellation Policy
Please provide at least 24 hours' notice to cancel or reschedule an appointment. Late cancellations and missed appointments may be subject to a fee.
Prescription Refills
Refill requests are handled during scheduled appointments. Please plan ahead, as same-day refill requests outside of a visit may not always be possible.
Controlled Substance Prescribing
Although my practice is primarily conducted via telehealth, federal prescribing laws currently require one in-person visit a year for controlled substances. I will discuss this with you on an individual basis.
Telehealth Consent
All appointments take place via a secure, HIPAA-compliant telehealth platform. By scheduling an appointment, you consent to receiving care by telehealth.
